You can apply online, fill out an application form or write us a letter, and send it to us by post, email or fax.

Making an application

Who is sending us the application? What do you need to know?

If you have appointed a person as your representative, you must let us know in writing (by email is fine). We will need their contact details to communicate with them on your behalf.

If you have a representative, we will communicate with them about your application.

You are encouraged to provide any further relevant information you have gathered, since the decision was made, with your application if it is available to you.

You will be able to provide further relevant information through the review process.

A friend or support person

Someone else can help you fill out an application form. They can send us documents on your behalf.

A representative

Your representative could be a lawyer, a disability advocate or a trusted family member or friend.

They should not have a financial interest in the outcome of the review.

If you have a nominee

If a plan nominee  has been appointed under section 86 of the NDIS Act, you should tell them that you have applied for a review of an National Disability Insurance Scheme (NDIS) decision with the AAT.

A parent or guardian of a child

You can fill out an application form on behalf of a child. You will need to tell us your full name, contact details and relationship to the child. You will need to provide the child’s full name, date of birth and address if it is different from yours.

If the child's parents are separated, you may be required to provide copies of any Family Court orders or Children's Court orders regarding the child.

You may be required to provide consent from the other parent for you to represent the child in the application.

Apply online

This is the best way to apply for a review. You can access our AAT Online Services 24 hours / 7 days a week. When you submit your application online you should also attach a copy of the National Disability Insurance Agency's (NDIA’s) latest decision.

You can also use our online system to submit additional documents later in your review.

Fill out an application form

Alternatively, you can find the application form on our website or you can ask us to send you one. When you submit your application form, you should also attach a copy of the NDIA’s decision.

You, your representative or another person can complete the application form. Our staff can answer your questions, but they cannot give you advice or tell you what to write. Our staff cannot complete the application form for you.

Writing to us

You can also make an application by writing to us. If you write us a letter or email, make sure you:

  • give us your name, postal address, telephone number and email address
  • tell us the date you received the NDIA’s decision
  • tell us why you think the NDIA’s decision is wrong
  • attach a copy of the latest NDIA decision. If you can't give us a copy of the decision, tell us in a few sentences what the NDIA’s decision said.
