USING THE FORMS

Our forms are available in PDF Format and Rich Text Format (RTF). You can:

  • print and fill out a form using either format, or
  • complete and save the RTF version of a form on your computer or tablet.

The RTF forms can be completed electronically using any word processing program, including Microsoft Word. You can complete the “Signature” field by typing in your name.

The three Application for Review RTF forms are designed so you can only enter details in the grey boxes. They expand in length as you are typing. You can get more lines in some fields by clicking the enter key. If the form does not allow you to type more information, type the extra text in another document and send it with the form.
